Before You Proceed with Filling Out the Form

Please Have the Following Information Ready Before You Start Filling Out the Form:

  • the name of the person who placed the order,
  • the email address you provided when placing the order.
    All correspondence related to the handling of your complaint will be sent to this address,
  • your order number. You can find it in the order confirmation email, on the invoice, or in your customer account under the “Orders” tab,
  • photos of any damage to the shipment and the Wavemaster products.
